Inhalt: Objective: the objective of this study is (re) thinking health actions transiting in that territory, and, in fact, such actions include the entire universe of feelings, doubts and uncertainties that permeate male thinking when it comes to the rectal exam. Method: the method is the design of test characterized as a reflective exercise on a theme, seeking a new way of looking at the subject. Results: masculinity is constructed by the influence of the social imaginary, and cannot ignore symbolic aspects that affect the decision to take the examination. This examination does not only affect the prostate, it touches on the symbolism of the aspects of what is "being a man". Conclusion: it is for health professionals to understand what happens in the "man's world" in order to think about health actions to reduce morbidity and mortality rates of this disease that has been affecting the health and quality of life of men.

Im reproduktionsmedizinischen Labor treten männliche Leiberfahrungen vor allem bei der Spermiengewinnung stark in Erscheinung. Mit der Spermienanalyse geraten der Männerleib und -körper wieder in den Hintergrund und bleiben auch bei der Zeugung weitgehend absent. Die Spermien selbst werden nach der Abtrennung vom Männerkörper zur vulnerablen Substanz, die eines besonderen Schutzes bedarf. Mit dem Verfahren der intrazytoplasmatischen Spermieninjektion bekommt das einzelne Spermium zudem eine bemerkenswerte Bedeutung und erhält selbst eine Art Körperlichkeit." (Autorenreferat)
Inhalt: "This article investigates the male body and the male lived body in the laboratory of assisted reproduction. The question is considered of the absence of 'the male' in medical analysis and whether the focus is only on the sperm. In the reproduction laboratory the lived male body emerges only in the context of sperm collection. Later, when semen is analysed, the male (lived) body disappears and does not appear again, not even in the fertilisation process. The sperms themselves become a vulnerable substance that needs special protection. In the context of intracytoplasmic sperm injection the individual sperm becomes remarkably significant and achieves a certain corporeality." (author's abstract)

Inequality in health care utilization in Germany? Theoretical and empirical evidence for specialist consultation

Inhalt: Aim: In view of increasing concern about a two-class system in the German health care sector, this study investigates the relevance of health insurance schemes and other socioeconomic characteristics to the level of specialist health care provision. Subjects and Methods: Referring to Ronald M. Andersen’s model of health care utilization and more content-based approaches, we implement a negative binomial hurdle regression to estimate the number of specialist visits within the last 12 months. Our data source is the German sample of the first wave of the Survey of Health, Ageing and Retirement in Europe (SHARE) in 2004. Results: The results show that men’s number of specialist visits is markedly sensitive to predisposing and enabling factors, whereas women’s health care utilization depends less on such socioeconomic characteristics. With reference to previous findings concerning general practitioner consultation, the assumption of a bipolar health care system providing general practitioner care primarily to the statutory insured and specialist care to the privately insured is supported empirically as to men. Education, which is considered to be highly correlated with health lifestyles, has a positive effect on medical health care. Every additional year of education increases by about 10% the probability of men seeking specialist consultation. Furthermore, the results indicate an unfavorable situation for the self-employed concerning health care because of their specific employment situation and health insurance coverage. Discussion: The research results suggest the existence of relevant differences in the amount of specialist consultation according to health insurance and other socioeconomic features. Further research could concentrate on the question of whether these inequalities in utilization levels indicate overprovision or underprovision of ambulant health care. Moreover, we recommend longitudinal research that is particularly suited to detangle age and cohort effects.

Dann werden die Datenbasis, die verwendeten Variablen und die Untersuchungsmethoden kurz beschrieben. Schließlich wird über die Ergebnisse der Ereignisanalyse berichtet, in der die Erwerbs- und Familienverläufe von Frauen sowie deren Gesundheitsverlauf als parallele Prozesse im Lebenslauf betrachtet werden, um damit die Zusammenhänge und Kausalitäten besser aufzeigen zu können.
