Inhalt: "Selbstbestimmung, informed choice und informed consent sind zu Schlüsselbegriffen in der Geburtshilfe avanciert. Dabei fällt die Popularität des gegenwärtigen Selbstbestimmungsdiskurses in eine Zeit, in der die Geburtshilfe im Zeichen der neoliberalen Ökonomisierung einem tiefgreifenden Wandel unterliegt, der sich u.a. in einer deutlichen Verschlechterung der strukturellen Rahmenbedingungen der Versorgungsqualität von schwangeren und gebärenden Frauen und in einer Abwertung von somatisch-beziehungsorientierten Momenten von Geburtsbegleitung zeigt. Der Beitrag untersucht, welche Bedeutungsverschiebungen das Verständnis von Selbstbestimmung durchlaufen und welche Effekte dies aktuell für schwangere und gebärende Frauen in der Geburtshilfe hat. Gezeigt wird, dass und wie der derzeitige Selbstbestimmungsdiskurs in der Geburtshilfe dazu beiträgt, die Verantwortung für das Gelingen einer 'guten' Geburt auf die Frauen zu verschieben und gleichzeitig den Verlust jener somatisch-beziehungsorientierten Bedingungen, auf die es für eine gute Geburtshilfe ankommt, zu legitimieren. Im Beitrag wird dafür plädiert, Selbstbestimmung nicht länger als Frage der Information, der Vorbereitung und der Entscheidung zu verstehen, sondern als Frage der Befähigung zu Urteilskraft, die Momente der Angewiesenheit, Achtsamkeit, Fürsorge, Schmerz, Angst, aber auch Kraft, Lust und Freude einschließen kann." Inhalt: "Self-determination, informed choice and informed consent have become important keywords in obstetric and midwifery practice. The popularity of the current discourse on self-determination coincides with the fact that obstetrics is increasingly being subjected to neoliberal economization and is thereby undergoing profound change. This change manifests itself, above all, in the fact that the structural framework of care for pregnant and birthing women is clearly deteriorating, and in that the somatic relationship-oriented aspects of obstetrics and midwifery are being devalued. I analyze the shift in our understanding of what self-determination is and I also map out the current effects of this shift on pregnant women and women in labour. Furthermore, I illustrate that, and in what way, the current discourse on self-determination in obstetric and midwifery practice leads to the fact that the responsibility for a “good” birth is being transferred onto women. This signifies a loss of the particular temporality and the somatic phenomena the woman in labour experiences. In my conclusion I argue that self-determination should no longer be understood as a question of information, preparation and decision, but rather as enabling judgement, which includes aspects of dependence, awareness, care, pain, fear, but also power, desire and joy." Inhalt: "This article presents the Germany-wide project Erzählcafés 'Der Start ins Leben' (Storytelling Café - A Good Start in Life) that was launched in 2014 as part of the political protest around birth and obstetrics that is ongoing in Germany. Its aim is to make the experiences of pregnant and birthing women, fathers, midwives and doctors visible in order to provide an alternative to the negative view of obstetrics in Germany. First, the political circumstances will be described. In a second step, the method and main results of the project will be presented. Finally, I discuss whether and how the project may be of use for bringing about political change as well as for developing new methods for the social sciences." (author's abstract)

Inhalt: Objective: To understand the practices and professionals involved in prenatal care in the Family Health Strategy. Methods: An integrative review of the literature with a temporal scope between the years 2001 and 2012, in the SciELO database and CAPES portal of journals, using the following descriptors: Women's Health; Prenatal Care; and Health Personnel. Results: It was observed that some articles emphasize the work of nurses over the multidisciplinary work, revealing the difficulty of a comprehensive team practice. Furthermore, the focus of care is almost exclusively technical, demonstrating that the most relevant aspects to the quality of the actions during prenatal care concern the number of consultations, tests and vaccines. Conclusion: It is essential that health professionals work in a multidisciplinary perspective, in which the care given to pregnant women happens is comprehensive and complementary.

Inhalt: Objective: to investigate the nurses' knowledge of gynecological Pap smears performed in pregnant women attending Primary Health Units (UAPS), Fortaleza-CE. Methods: a descriptive, cross-sectional study conducted in three UAP, with a sample of 27 nurses. Data were obtained through a questionnaire and the analysis used was the Epi-info program. Results: the data showed that the majority of nurses, 17 (62,97%), does not perform the gynecological examination in pregnant women. From nurses who perform, 3 (7,4%) make the collection incorrectly. About participating in trainings on prenatal and gynecological examination, 24 (88,8%) reported previous participation. Conclusion: thus, systematic and effective training aiming to overhaul the welfare practices that are established in the family health programs are necessary.

Inhalt: Objective: To understand the meaning of a humanized childbirth in the conception of pregnant women. Method: This was a descriptive study of a qualitative nature with a phenomenological approach. Semistructured interviews were conducted with 15 pregnant women enrolled in an outpatient prenatal care at a university hospital in the city of Montes Claros/MG. Results: The central unveiled theme was: Understanding the humanized childbirth as one in which the professional has as prerogative the empathic relationship and technical expertise enabling the woman to experience labor and delivery as the physiological protagonist during the process. Conclusion: The pregnant women define the humanized childbirth based on the philosophical bases of childbirth humanization and birth advocated by the Ministry of Health, which has the interpersonal relationship and competent assistance as principles.

Inhalt: Objective: analyzing the values expressed in the speech of women/mothers in the accessibility of monitoring examinations of prenatal care. Method: a phenomenological, descriptive, exploratory study with a qualitative approach. There were interviwed fifty women/pregnant of the Prenatal Program of Niteroi, Rio de Janeiro, Brazil. The data analyzed allowed formulating themes articulated with the Theory of Values. Results: barriers experienced by women on laboratory/images examinations: delay in results, misinformation and lack of a service of quality, a value considered of vital use for them. We realize the need for change in monitoring prenatal, considering a network of unified and integrated health with quality services at different levels of care, promoting wellness, comfort and safety to pregnant women as a vital value to their health. Conclusion: the health network should be unified for a better quality of services offered to women.

The occurrence of pre-eclampsia in women pregnant for the first time attending prenatal care consultation at a university hospital

Inhalt: Objective: To investigate the characteristics of pregnant for the first time attended at prenatal; identify the incidence of preeclampsia in pregnant for the first time attended consultations in prenatal care. Method: A descriptive, quantitative, retrospective, held at a University Hospital in Rio de Janeiro. The charts of women pregnant for the first time attended from 2008 to 2009 were analyzed with descriptive statistics support. Results: There were 264 (56,3%) admissions of pregnants for the first time. Were part of the sample set 105 records. In this group, 43 (40,9%) women showed in the edema, 2 (1,9%) had proteinuria, 29 (27,6%) had no abnormalities of blood pressure measurements. Conclusion: In the analyzed sample there was no record of preeclampsia, however 41 (38,9%) women showed changes compatible with BP Disease Specific Hypertension in Pregnancy (HDP). Nurses have an important role in the multidisciplinary team, for early detection of complications during pregnancy contributes to reducing the incidence of maternal mortality and morbidity.

Inhalt: Objective: To identify the socioeconomic and behavioral profile of pregnant women cared for in prenatal nursing consultations at a basic health unit in the State of Rio Grande do Sul, Brazil. Method: Quantitative, documental and retrospective research. Eighty-eight nursing records were collected from medical data of pregnant women cared for from January 2009 to November 2010. Descriptive statistics was used for data analysis. Results: The predominance was: young pregnant women, married, housewives, with low level of education; family incomes of up to two minimum wages; basic sanitation available; no consumption of alcohol, cigarettes and drugs; diversified diet; restricted physical activity; regular dental care; need for reinforcement of tetanus immunity; first prenatal consult with less than 20 weeks; and planned pregnancy. Conclusion: The identification of pregnant women's characteristics allows identifying their main needs. This could be used for promotion of health by improving prenatal care.
