Inhalt: Healthcare has long been a gendered enterprise, with women taking responsibility for maintaining health and engaging with service providers. Universal healthcare provision notwithstanding, women nonetheless undertake a range of healthcare work, on their own account and on behalf of others, which remains largely invisible. As part of a multi-method comparative European study that looked at access to healthcare in diverse neighbourhoods from the point of view of people's own health priorities, the concept of "healthcare bricolage" describes the process of mobilizing resources and overcoming constraints to meet particular health needs. Bricolage mediates between different kinds of resources to meet particular challenges and describing these processes makes visible that work which has been unseen, over-looked and naturalised, as part of a gendered caring role. Drawing on 160 semi-structured interviews and a survey with 1,755 residents of highly diverse neighbourhoods in Germany, UK, Sweden and Portugal, this article illustrates the gendered nature of healthcare bricolage. The complex variations of women's bricolage within and beyond the public healthcare system show how gendered caring roles intersect with migration status and social class in the context of particular healthcare systems.

Dabei fällt die Popularität des gegenwärtigen Selbstbestimmungsdiskurses in eine Zeit, in der die Geburtshilfe im Zeichen der neoliberalen Ökonomisierung einem tiefgreifenden Wandel unterliegt, der sich u.a. in einer deutlichen Verschlechterung der strukturellen Rahmenbedingungen der Versorgungsqualität von schwangeren und gebärenden Frauen und in einer Abwertung von somatisch-beziehungsorientierten Momenten von Geburtsbegleitung zeigt. Der Beitrag untersucht, welche Bedeutungsverschiebungen das Verständnis von Selbstbestimmung durchlaufen und welche Effekte dies aktuell für schwangere und gebärende Frauen in der Geburtshilfe hat. Gezeigt wird, dass und wie der derzeitige Selbstbestimmungsdiskurs in der Geburtshilfe dazu beiträgt, die Verantwortung für das Gelingen einer 'guten' Geburt auf die Frauen zu verschieben und gleichzeitig den Verlust jener somatisch-beziehungsorientierten Bedingungen, auf die es für eine gute Geburtshilfe ankommt, zu legitimieren. Inhalt: "Self-determination, informed choice and informed consent have become important keywords in obstetric and midwifery practice. The popularity of the current discourse on self-determination coincides with the fact that obstetrics is increasingly being subjected to neoliberal economization and is thereby undergoing profound change. This change manifests itself, above all, in the fact that the structural framework of care for pregnant and birthing women is clearly deteriorating, and in that the somatic relationship-oriented aspects of obstetrics and midwifery are being devalued. I analyze the shift in our understanding of what self-determination is and I also map out the current effects of this shift on pregnant women and women in labour. Furthermore, I illustrate that, and in what way, the current discourse on self-determination in obstetric and midwifery practice leads to the fact that the responsibility for a “good” birth is being transferred onto women. This signifies a loss of the particular temporality and the somatic phenomena the woman in labour experiences. In my conclusion I argue that self-determination should no longer be understood as a question of information, preparation and decision, but rather as enabling judgement, which includes aspects of dependence, awareness, care, pain, fear, but also power, desire and joy." Inhalt: Objectives: To know the contribution that groups of pregnant women have upon building knowledge about the process of parturition. Methods: A descriptive-exploratory study, with ten women who participated of groups of pregnant women. The semi-structured interviews were recorded and transcribed. The analysis was based upon thematic analysis, after approval by the Research Ethics Committee-CAAE 20722913.7.DO00.5317. Results: From the analysis of the transcripts emerged the themes: Labor and delivery from the perspective of women who participated in the groups of pregnant women; and Childbirth Humanization and Birth: Knowledge vs. Experience. Conclusion: The group of pregnant women allows women to prepare for the delivery process, as an information and exchange experiences with other participants and health professionals provide input for their choices, and thus, knowledge is built on a reciprocal basis, in a trusting and learning environment.

Inhalt: Objective: To identify the prevalence and the profile of pregnant women victims of physical violence. Methods: Quantitative, cross-sectional study, conducted with 105 pregnant women followed in four Family Health Units at the city of Jequié, Bahia. The data were collected through questionnaires from the Abuse Assessment Screen (AAS); Tolerance, Annoyed, Cut down and eye opener (T-ACE) and sociodemographic. Results: The prevalence of pregnant women who suffered physical violence was 2,9%. These were 21-26 years of age, race/ skin color black (black and brown) and suffered aggression on the head predominantly by the partner who used alcohol. Conclusion: The prevalence of pregnant women victims of violence was not high, but it is an issue that causes many negative impacts on the woman's and baby's health. Thus from the profile of the women surveyed, it is recommended that specific actions could be carried out in order to at least minimize the occurrence of this problem.

Inhalt: Objective: To understand the factors that influence the process of choosing the type of delivery, from the perception of mothers. Methods: This is a qualitative and descriptive research, held at the Women's Hospital Midwife Maria Correia, Mossoró/RN. A total of 14 mothers were interviewed. Data were analyzed according to the Collective Subject Discourse technique. The study was submitted to the Ethics Committee in Research of Universidade Potiguar approved under the number CAAE: 38520214.0.0000.5296. Results: Women's desire a birth delivery without complications and fast recovery. The influences of the "fear of pain" and individual experiences and other women for choosing the type of delivery are significant. Conclusion: We emphasize the importance of access to prenatal quality actions that provide safe choices, clarifying doubts and future mother's wishes, reassuringly for delivery.

Inhalt: Objective: To understand the practices and professionals involved in prenatal care in the Family Health Strategy. Methods: An integrative review of the literature with a temporal scope between the years 2001 and 2012, in the SciELO database and CAPES portal of journals, using the following descriptors: Women's Health; Prenatal Care; and Health Personnel. Results: It was observed that some articles emphasize the work of nurses over the multidisciplinary work, revealing the difficulty of a comprehensive team practice. Furthermore, the focus of care is almost exclusively technical, demonstrating that the most relevant aspects to the quality of the actions during prenatal care concern the number of consultations, tests and vaccines. Conclusion: It is essential that health professionals work in a multidisciplinary perspective, in which the care given to pregnant women happens is comprehensive and complementary.

Inhalt: Objective: Descriptive research, with a qualitative approach, aiming to know the companion's perceptions about the organization and ambience of the obstetrical center, and to identify which aspects facilitate and make difficult their stay. Method: Data was collected through semi-structured interviews with 16 companions chosen by delivering women from November 2010 to May 2011, in a public maternity in Santa Catarina, Brazil. Results: Using Collective Subject Discourse in analyzing the data, the following four themes emerged: orientation about norms and routines supplied to the companion before entering the obstetrics ward; obstetrics ward ambience; aspects which facilitate staying; and difficulties surrounding staying in the obstetrics ward. Conclusion: although some difficulties were faced, especially resulting from the lack of orientation and inappropriate accommodation of the companion, they were not obstacles to their permanence by the delivering woman's side.

Inhalt: Objective: To analyse the women's perception on the violence experienced. Method: Qualitative study. Data was obtained through a semi-structured interview, and was analysed through the Content Analysis modality. Results: Four thematic categories were identified from the analysis of the accounts of the 14 participating women: 34,8% of the accounts fit in the category Perception and Feelings about violence; 26,1% presented accounts on the category Perception of the Motives for the Experience of Violence; and also in the category Freedom Cessation; and 13,0% in the category Reflex of Violence on Health. Conclusion: There is a need for policies and actions that support and contribute to women's freedom from violence, as well as the strengthening of intersectoral support networks, as violence poses a number of problems not only of socio-economic nature, but also damages the health of their victims.
Quality of life of mastectomized women enrolled in a rehabilitation program

Inhalt: Objective: To evaluate the quality of life in mastectomized women enrolled in a rehabilitation program, according to sociodemographic and clinical variables. Methods: This is a descriptive study, conducted at the Rehabilitation Program for Mastectomized Women, at Espirito Santo, Brazil. The sample consisted of 53 women interviewed according to the standards of Visual-Analogue Quality of Life Scale for Head and Neck Cancer Carriers adapted to Breast Cancer Carriers with the consent of the authors. The data received statistical analysis by SPSS version 17.0 software and sociodemographic variables were assessed using Pearson's correlation coefficient. Results: It was found that younger women without breast reconstruction married and submitted to chemotherapy or hormonal therapy had greater loss of quality of life. Conclusion: Therefore, there is the importance of interventions to promote a better quality of life for these patients, with effective interdisciplinary action by the health team.

Inhalt: Patients' viewpoints are commonly used to assess quality of care in diverse healthcare organizations. This permits managerial decisions to be made based on knowledge rather than conjecture. The purpose of the current study is to investigate quality gap of family health care through measuring differences between clients' perceptions and expectations at Kashan city health centers in Iran.Methodology: A cross-sectional design was applied in 2013. The study sample was composed of 384 women clients of family health services randomly selected from Kashan city health centers. The SERVQUAL questionnaire was used for data collection. Service quality gap was measured by computing the difference between the rating respondents assign to expectations and perceptions statements. For pair wise comparison of the expectations and perceptions the Wilcoxon Signed Rank test was used. Internal consistency of the scale was confirmed with Cronbach's Alpha coefficients (.81 for expectations and .84 for perceptions).Findings: In all dimensions of the services a significant (P<0.001) negative quality gap was observed. The maximum quality gap was in the tangibility (-0.60±0.37) and the minimum one was in the responsiveness (-0.41±0.31) dimension. The findings of the research demonstrated that there was negative gap between clients' expectations and perceptions in all dimensions of the services. Thus, family health care managers should apply improvements in all five dimensions of the services.
